Mobile Intake Needs

Hello! We are just getting started with Mobile Intake and have a few needs. We ask for alot of information from our clients - and they don't mind giving it. Couple of needs to round out our intake process:

1) We ask for the Client's children - this helps us close cases of previous clients and gives us a picture of the client's needs. There is no place in the current mobile forms to gather this. We would either have to create a separate paper form or try to answer it during a session, which is unnecessary and take up valuable client time. Is there a way to add this?

2) We ask specifics regarding abuse - clients who are victims of abuse often do not self identify as a victim. So we ask specific questions: in the past year, has anyone ever threatened you or made you feel afraid?, has anyone ever hit, slapped, kicked, choked or physically hurt you?, Has anyone forced you to do something sexually that made you feel uncomfortable?, etc. Is there a way to ask this? If not, we will need to create a paper form or attempt to create an HTML in the consent forms.

3) In our area, there is a common practice of using HCG injections or drops for weight loss. we specifically ask this question on our intake forms in order to prevent false positives. Any chance you could add this to the pregnancy test form?

4) We use a form for clients to consent to the release of confidential information to someone they request. Is there a way to create a consent form for this where the client enters information? I can probably create a some HTML code, but wondered if someone had already done it.

5) For STI/STD testing, we ask very specific questions regarding their potential risk of contracting STDs. They are gender specific. Is there an opportunity to expand the health questions or create a new form for intake?

6) Is there a way for a login to CenterPiece to be read only? we want our advocates to be able to see information for the clients but not be able to change it.

It's great to hear from you! I would encourage you to reach out directly to your account manager so we can gather all the details to record your feedback to our feature request list to be considered fur a future update as well as talk about steps of how you could possible do this today (creating a 1 page form with "additional questions" and then scanning/uploading to the Docs tab).
