I would like to make some suggestions to add to the assessments.
Under the pregnancy test LMP, a approximate button might be nice, or even a unknown. Under same assessment. Selection for none under Birth control.
Under the health history, change the writer from plugging in the name of the person that triggers case. We have volunteers that enter case information,a lot of times. I think that it should be a nurse putting in that history.
And under sonogram report. What is the official defination of inconclusive. I have an idea of what I thnk it means but would like your input.

It's great to hear from you, thank you for taking the time to share your feedback with us and your sweet encouragement! I will address each item below. If you have further questions I encourage you to reach out to your Account Manager directly.
LMP: Currently you are able to leave LMP blank and in your chart note you can specify whether it is an estimate or not. How would knowing this date was estimated assist you?
Birth Control: If you do not check a box the it is reflected as "Not on Birth Control" however I will add the request of "None" be added
Health History: The Staff/Volunteer should be the person who reviewed the form and gathered the information with the client-- not the originator of the case.
Sonogram: At my center we determined "Inconclusive" if there was no viable pregnancy confirmed meaning the sonographer was not able to detect/measure a heartbeat.
