Pregnancy test forms

Has there been an updated pregnancy test form? Today when I was documenting pregnancy tests, I noticed there were new options for questions 4&5. I could not find any updated pregnancy forms in ekyros, so I am a little confused.

Yes, we updated Q4 with a few more options. See below. Q5 has not changed. Let me know if you have any other questions. Blessings!
New Q4 options:
Headache, Moodiness, No Period, Other and Vomiting. |

Hi Mary Lou,
Just to add to what Brian said we have indeed updated the Pregnancy Test Form for data entry, as well as the printable office form for use in the consulting room that can be found in Client Management under Office Forms. I hope this helps!
