Care Net Report- FOB and Exit Survey

Hello there,

Can you please tell me if there are plans to incorporate the newest STAT requirements from Care Net into a client form so that we an track this?
1. Relationship of FOB to the at-risk positive test client
2. Exit survey questions

Hi Laura,

Yes, we do plan to add these to our forms shortly so you can begin tracking them for your 2015 reporting (for the 2014 year). I hope this helps!

I've seen the place for FOB. Where will the evaluation question be? Will it be incorporated with each "case visit" or a separate report?

Hi Georgia,

We are still working on the best way to implement the Care Net question about the evaluation of the client's experience at the center. From what I've read of Care Net's instructions, the survey is potentially given at each visit, so you will likely be able to enter multiple forms for each client.

Thanks Amy,

Yes, we are doing the survey at the end of each appointment. I currently don't track who the counselor is. (Frankly that takes too much time and we can always look it up in the file if we ever need the info.) Do you plan to ask who the counselor is?

Hi Georgia,

Probably not. The main goal is to track the main question that needs to be reported to Care Net. Do you put the surveys back in the paper file now? I know per Care Net's instructions it is meant to be entirely anonymous, but I also know some centers prefer to keep it tracked back to the specific client. We're trying to get our heads wrapped around the different scenarios. :)

Currently the plan is to track the survey by Counselor and keep the client anonymous. In other words you don't attach the exit survey to a client file. It's our understanding that the purpose of the survey is to document the quality of service provided by your staff. Does that make sense?

Excellent
Good
Neutral
Fair
Poor

Currently, we are just putting them into a large envelope. We've started just so our counselors can get used to it. The client does not put their name on it and she fills it out while her next appointment is being made.

The question is, where will that info be entered? If it's recorded with each visit, then it WILL be attached to the client's file. There's no way around that. We will also need to put it in her file so that I can see it.

I guess you could create a daily or weekly report that is completely separate from the files. Would the number of surveys have to match the number of visits? Sometimes it takes 3 or 4 days for the file to make it's way around to me. I don't know that I could record it any other way than to enter it along with the visit information. That takes away the anonymity.

Oh well.... good luck with it... LOL

Hi Georgia,

The last we talked about it, we were hoping to give the option to either tie directly to the client file or to leave it completely anonymous. It's a work in progress, but certainly a main goal is to adhere to Care Net standards since it is for the primary purpose of reporting to Care Net. We appreciate your feedback and letting us know how you use the survey. Stay tuned. :)
