I searched for a similar error but did not find one.
This is very intermittent. It happens on both Windows releases XP & Version 7. It has occurred on Firefox, Chrome and IE. I have seen it SO MANY TIMES that I have tried to identify my actions. I notice nothing that I do special to cause it.
Using the quickform (that's all I use) when I get to the option to select (check) pregnancy test or sonogram (either one has exhibited this issue):
1. After the check is made to the box, the new page opens automatically
2. I press the tab key, most of the time the cursor is placed into the first active field
3. The error that I am reporting is that sometimes, after the tab key is pressed, the screen is positioned to the top of the file/record and I have no idea where the cursor is. If I press tab again, there is no obvious cursor movement. I can't find the cursor. At that point, I manually move the screen, via the cursor key or the mouse, back to the form for data entry and continue.
I doubt that you can reproduce it since it is intermittent. Someone will have to look at the code at the point of after the 'new' screen is opened for pregnancy test or sonogram to see why the screen would be placed back to the top of the file. For me it is a time waster to have to re-position the record.

Really strange....today, I have been able to reproduce the problem. I have tried multiple times and can make it fail every time. This is the reason it is intermittent for me. I seldom use the mouse to position the cursor to the fields.
This is a working scenario:
1. I have just completed the pregnancy form.
2. I 'tab' to the selection - sonsogram - and press the space key. This selects the form.
3. The form opens. I tab to the active field to enter the name. It works. Everything is fine.
This is the failing scenario:
The operation is different. I have used the 'mouse' to move through the Pregnancy form.
1. I press the left mouse key to select the sonogram form.
2. The form opens. I use the tab key to select the active field (staff/volunteer name)
3. The screen is positioned to the top of the record. I have not been able to locate the cursor at that point. A continuous tab does not effect any movement of the cursor. I use the mouse to re-position the screen to continue. I can enter data.
4. I uncheck the form. The sonogram record will close (without saving).
5. At that point, I can reproduce the problem by the previous activity order.

By the way - I tried the same mouse actions to enter a pregnancy form and it will fail the same way....re-produceable. |

The same issue occurs on the Client Intake Form CASE information page when the "what outside help are you receiving' is opened by use of the mouse left-click and you attempt to TAB to a field in the opened view/box. The screen is refreshed to the top of the record - as on the previous update.
But, I can find the cursor now. It is positioned on the first field of the record. If one hits the TAB enough times it will be positioned into the record eventually.
Glad I found this error - at least, I'm not sitting scratching my head trying to figure out what did I just do?
