Hi Patricia,
Thanks for posting on the forums. Your second stage of At-Risk Assessment is tracked on the Pregnancy Test form. So you can the find the person's Pregnancy Test form and edit it at the very bottom to edit that portion. I hope this helps!

Hi Patricia,
To add to what Amy stated I wanted to provide a breakdown of each stage.
Stage 1-- gathered from the Case Form-- tracks INITIAL (before services are provided) at risk assessment and client's stated intentions
Stage 2-- gathered from the Pregnancy Test Form-- tracks AFTER SUPPORT (after pregnancy test has been provided and services shared) at risk assessment (staff/volunteer) and client's stated intentions
Stage 3-- gathered from the Sonogram Form-- tracks AFTER SONOGRAM at risk assessment and client's stated intentions
Stage 4-- gathered IF the client's assessment/stated intentions have CHANGED since the last recorded stage, at risk assessment and stated intentions
