Typo's on Initial Visit Form Template

It's not a problem at all because we've downloaded and customized these forms, but FYI, on the Initial Visit Form "Mormon" is misspelled and an "r" is missing from the question, "What is your annual income?"
We love CenterPiece! Thanks for this great product!
