Pregnancy Test Form

I spoke with Terri M and she is asking if we can add "Not Sure" or "Unsure" as a possible answer for questions 17, 18, and 19 on the Pregnancy Test Form.
Right now the possible answers are: Yes, No, and Unknown. If a person answers "Unknown" it is assumed that the question wasn't asked and the volunteer doesn’t know, but if the question is asked and the client says she doesn't know if the father of the baby will be involved, they would like to select "Unsure" or "Not Sure" as the answer.
In addition, she is asking if we can add the option of "Unknown" to questions 6 - 13 on the Pregnancy Test Form. If they are entering older client files, they don't have this information and want to be able to select "Unknown" instead of "Yes" or "No".
