Why not an intial visit for each new case? How does the information in the initial form get updated for each new case? The marital status and education status could change.

Hi Patricia,

FYI: The system allows you to enter an initial visit form for each case. The marital status , education along with all the demographic information is also tracked on a case by case basis. What exactly are you asking?

I have been told that you only enter an initial for a client one time. I don't understand why you wouldn't enter an initial visit for each time the client comes in for a new pregnancy test. It seems like there should be an initial visit for each new case. I guess my question would be, when do you enter an initial visit?

Hi Patricia,
It IS appropriate to enter a NEW Initial Visit Form for each NEW case. This is actually how the system is designed to be used. So your assumptions are absolutely correct! :)

I do know that some centers have chosen to only enter one Initial Visit form. While this is ok to do, eKYROS recommends that you enter a NEW Initial Visit form for each NEW case (1 per case).

Sarah,
How would you record a second 'initial visit?' When I pull up her record, the info from the first is there. If I change that, I lose previous data.

Hi Georgia,

The case form allows you to record "case intake" information one time. Changes to the case intake information shouldn't happen too often, but if they do you should update the case and if you want to track the previous data, then one possible option is to record the change by adding a note.

Please note, you can track multple cases per client file, so this should be a problem if the information changes between cases....as the history would be preserved on the prior case.

Hi Georgia,
Previously the "Initial Visit" form was separate, now it is part of the "Case" form. A client "case" is simply a support cycle; it's a group of activities around a specific event, such as pregnancy or post abortion counseling. A case as defined by CareNet is "a situation involving any client who has NEVER been to the center or a RETURN client with a new possible pregnancy related concern..." For each new case you will enter a new case form, capturing the details of that time while still maintaining the previous case and data from that support cycle.
