New Case?

I have a client who came in for a test on 2/15/06, then came back on 3/9/06 and she had been to the doctor and they told her she was positive. Would I give her a new case number, yes correct? Also what forms would I assign to her. pregnancy test or pregnancy history? THANK YOU FOR YOUR HELP!

Hi Ashleigh,

Great question.

If this is the same pregancy related case, you would *not* want to create a new case. Instead, you would add a new pregnancy test to the exisiting case. If the first test was not conclusive, you would want to mark the test result "inconclusive". On the second test, if the client can get a note from the doctor to prove she is pregnant, you would mark the 2nd test result "postive" and check the "Proof of Pregnancy (PoP) checkbox. (The stats break out postive tests by "positvie and positive (Pop)".

this way, you have a record of the services your center provided (1st test) and the Proof of pregnancy on the second test.

The first test was negative, she went to the doctors a few days later had a test and it was positive. So I mark PoP, but how do I still use all the information from the previous intake form?

I'm not quite sure what you mean, but you should have an intake form associated to the case. both tests should be associated to the case. the intake information applies to the entire case.

If the same client comes in for parenting classes after having come in for a pregnancy test, is it a new Case number or just a new case visit?

Assuming the parenting class has to do with the same preganncy related concern (from the prior visit), i would assign it a new case visit versus creating a new case.

Hi Andrea,

I totally agree with Brian. You'll only want to create a new case if there is a NEW pregnancy. All follow-up visits after her pregnancy test like options counsleing, classes, Earn While You Learn, etc. should all be included within the SAME case. If she comes back to you in a few years and is pregnant again, that would be considered a new case.

Okay, I'm a little confused. What if a client comes in who was a previous EWYL client or even a previous test client and needs a pregnancy test but it is negative. This is not a new pregnancy so should it be a new case number? If so, I'm assuming all subsequent case visits would go under the latest case number even though we are still dealing with same children that we were servicing in case #1?

Hi WendyStone,

Sorry for our delay in getting back with you. Our recommendation is that no matter what the actual pregnancy test result is (positive, negative, or inconclusive), you will want to start a new case every time a pregnancy test is done. THERE IS 1 EXCEPTION to this, and that is when a client has a "retest"--meaning that she has NOT had a period since the last time she had a pregnancy test. If it is a "retest," this should info be included in the same case as her initial test.

So, in your above mentioned scenario, when a client who has previously been in for EWYL or other services comes back again at a later date with a NEW pregnancy concern and gets a pregnancy test--YES, you will begin a new case. One case should include all the visits in her "cycle of support" (including the pregnancy test whether positive or negative, EWYL, abstinence counseling, etc., etc.). When a NEW cycle begins (with a new pregnancy related concern, post-abortion counseling, etc.) then you will begin a new case.

Hi, Sarah. We're having a discussion about the same issue. In the above mentioned scenario, Becky (Case 001) has previously been in for EWYL classes. She then gets a pregnancy test which is negative (Case 002). She continues with her EWYL classes. Will that be under 001 or do all of the activities now get reported under 002?

Another example. Susan (001) is getting diapers for a baby she already has. She comes in for another pregnancy test (002) which is positive. She continues to receive diapers for the baby she already has. Is that activity still reported under 001, or does all of the activity now get reported under 002?

I am loving your questions! Thank you for your attention to detail.

In the scenario with Becky you are correct. Previously Case 001 was her EWYL. She comes in for a new pregnancy related concern, which would be a NEW CASE (002). Regardless of test result (positive, negative, or inconclusive) all subsequent visits would be recorded under case 002. A “Client Case” is simply a support cycle; it is a group of activities around a specific event, such as pregnancy or post abortion counseling. A case as defined by Carenet is “a situation involving any client who has never been to the center or a return client with a new possible pregnancy related concern (new pregnancy, post abortion, abstinence).”

In the scenario with Susan upon her coming in for a pregnancy test you would close case 001 and open case 002. All subsequent visits would go under case 002 until she has a new possible pregnancy related concern.

I was trolling for another question and I found this and wanted to respond.

The way I've been doing it is if the second preg test is negative, I close that case (#2) and continue entering EWYL visits on case 1. Is there a problem with this? I suspect not since the number of cases is the same. It makes more sense for me this way since the visits (after the negative test) DO come from the first case. If the second case is positive, yes, I do add all subesquent visits to case 2.


My new question perhaps arises from us recently going medical.

Scenario: Client comes in on 1/3 and does a self-administered preg test which is positive. Client returns on 1/5 b/c she needs pregnancy verification for other agencies. How do I enter the second (medical) pregnancy test? It's not REALLY a retest since the first test was positive. But that's how I'm inclined to enter it. I could just ignore that fact that another preg test was done, but that's not acknowledging that resources were used. I guess it could be an OTHER....

It wasn't an issue when we weren't medical, b/c we could not do verifications and sent them to a doctor's office. We are a CareNet center, FYI.

I'd appreciate your thoughts,

We enter the second test as a re-test, because it is confirming the first one and, as you said, resources were used. We don't have many of those cases, but when we do I simply put retest in the form, but don't fill out the entire form. Be aware that when you do this you have to manually put the due date in. If you don't, when you run 'due date' reports, it automatically uses the lastest test, so if it's not on the second one, it won't show up in the report.
