Forms that match the database

We have noticed as we enter the information from the forms into the online database that the flow of the forms to the webpage does not coincide. This causes a lot of flipping of pages to get the information. This slows down the entering of the information considerably.
We are missing information on the forms also:
PGT form we need due date and weeks preg. This allows us to share this information with the client directly from the and as we counsel them.
PGT form we need the age of the potential birth father, for statistics and to report statutory rape.
Client intake form we need in the personal information next to birth date, age of client. (saves time)
Initial Visit forms we need a NONE and NO box for Do you have any STDs and Victim of abuse.

Hi Cindy,
thanks for the feedback. We are going to address the forms issue in an upcoming release. We have received a lot of feedback about this. I'll keep you posted when i have more info.

i just want to give you and update on your feature requests. feel free to reply to this thread out on the forums is you have additional comments/questions. The following enhancements will be included in the next CenterPiece update due to be released Sunday night (8/1). To learn more about this release, check out [Product Updates] in the forums. Click on the 8/1/04 thread.
My comments are in ALL CAPS inline with your original post.

WE ADDED THE ABILITY TO CONFIGURE THE OFFICE FORMS (SUCH AS INITIAL VISIT AND PREGNANCY TEST FORM) TO INCLUDE THE OFFICE USE ONLY QUESTIONS. THIS WILL MAKE THE FORMS MATCH THE DATABASE FORMS TO EASE DATA ENTRY. YOU WILL WANT TO PRINT NEW COPIES OF THE FORMS ONCE WE RELEASE THE SOFTWARE. CONFIGURING THIS OPTION WILL REMOVE THE INITIAL VISIT AND PREGNANCY TEST SECTIONS FROM THE OFFICE USE ONLY FORM. (ALSO, THE OFFICE FORMS HAVE BEEM MOVED BACK UNDER THE CLIENTS TAB)

WE ADDED A DUE DATE FIELD IN QUESTION #1. DURING DATA ENTRY, THE FIRST DAY OF LAST PERIOD CAN BE CALCULATED FROM THE DUE DATE.

THIS HAS BEEN ADDED.

CLIENT AGE CAN BE CALCULATED OFF THE BIRTHDATE DURING DATA ENTRY. WE CAN POSSIBLY ADD THIS TO THE FILE SUMMARY REPORT, BUT PROBABLY WONT ADD IT TO THE CLIENT CONTACT FORM. IF YOU FEEL YOU REALLY NEED THIS, PLEASE PROVIDE MORE INFORMATION.
#5. Initial Visit forms we need a NONE and NO box for Do you have any STDs and Victim of abuse.
THIS MAY BE A TRAINING ISSUE, THE FACT THAT NO CHECK BOXES ARE CHECKED, APPLIES NONE OR NO.
