Understanding the purpose of the Case Form, there are some things on the Initial Visit that I wonder might be better on the Case Form. As I was updating information on a young client yesterday, I noticed that some of those items on the Initial Visit she entered one way a year ago, she is entering an entirely different way this time.
E.g. living arrangements, outside help, victim of abuse.
Most items on the Initial Visit form are constant, unchanging. The above mentioned items (living arrangements, outside help, victim of abuse) are changing from pregnancy to pregnancy, or from one crisis situation to another. It might be helpful for us to know case to case the level of support, living arrangemtnts and if they are now a victim of abuse and were not previously. I'm sure most pregnancy centers could share similar sentiments - our clients move from boyfriend to boyfriend, fiance to fiance.
Brian, this is completely my opinion. If other centers do not share the same concern, please disreguard.

Thanks for the feedback. You are right, those items probably do need to be reviewed with every new pregnancy and would make sense to put on the Case form. I'll pass this along to development!
