Brian,
I was trying to update our forms today so they would correspond with the new Centerpiece updates. I successfully changed the Client Contact Form, but I had a question regarding the Pregnancy Test Form and the Office Use Only form. The PT form didn't look complete, and there was no where on either the PT form or the OUO form for the test outcome (+/-), due date, assessment of intentions (is that on the other form?). Do I just need to cut and paste from my old PT form onto the new one? I hope this makes sense, I feel really confused with all this new information.

Brian -
I noticed the same thing - it looks like questions #10 through #19 are missing.This information includes: drug/alcohol use, stress, pregnancy due to rape, pregnancy intentions, partner information and hypothesized involvement.

Great eyes! Thanks so much for catching this. It does look like questions 10-19 were mistakenly omitted from the Pregnancy Test (paper) Form. We will get this corrected.
Caroline, the other questions that you were having trouble finding (test result, due date, assesment of intentions) are found at the bottom of the Pregnancy Test Form, in the bottom section. I just double checked this, and they are all there.

Hi Caroline,
Depending how your pregnancy test form was configured prior to the v5.1 release, in some cases the OUO pregancy test information (test result, assessment intentions etc..) may not be displayed on either form. We are releasing an update tonight to fix this problem. In both the OUO and the Pregnancy test form, we are adding a checkbox that will allow you to include/exclude OUO pregnancy test questions.
