Pregnancy History - child's whereabouts

A client file recently peaked my interest into the information recorded into the "Pregnancy History" portion of the Client File. She has had 10 pregnancies; 1 child in the custody of someone else, 1 death, multiple adoptions and miscarriages (that can be registered) and 1 child that has been removed by DCF.
To me, this information is incredibly imporant to those counseling with her. If we loose the paper trail, the computer is the most concise way of retreiving the information - especially in the 'Pregnancy History' form.
I would love other feedback from other centers. Would it be possible to add a note category to the Pregnancy History form within each entry? (over the right of all the information).This way, we could have specific information regarding the child without having to fish through the notes or correspondence. Most of our clients don't share the names of the children.
