Hello
I have an issue with the health history form: Gravida(#prior pregnancies)

Gravida includes current pregnancy. If a nurse is filling this out and the patient has a positive pregnancy test then the gravida would include that pregnancy. This is confusing to our director because it says "prior pregnancies". If the patient fills this form out in some centers that would be fine. The incorrect use of the word "gravida" is the problem.
