1.Tracking Spiritual Decisions

Is there a report where we can see how many clients have actually accepted Christ at the Clinic?

I've found their spiritual status, but I don't see anything that says how many clients actually made a decision this month.
2.Typo's on Initial Visit Form Template

It's not a problem at all because we've downloaded and customized these forms, but FYI, on the Initial Visit Form "Mormon" is misspelled and an "r" is missing from the question, "What is your annual income?"
