1.Missing Staff/Volunteer names under activity

Recently I have noticed when going back into a client file to add or update info, the Staff/volunteer names are missing. It seems to be mostly Concepts Shared, Referrals and Health History. I have not had a problem with this in the past so I'm curious if anyone else is and what is the cause. All the other info is saved, just the name of the staff or advocate is missing.
