Health History Form
Posted: 04 Sep 2018 04:46 PM
Hello
I have an issue with the health history form: Gravida(#prior pregnancies)

Gravida includes current pregnancy. If a nurse is filling this out and the patient has a positive pregnancy test then the gravida would include that pregnancy. This is confusing to our director because it says "prior pregnancies". If the patient fills this form out in some centers that would be fine. The incorrect use of the word "gravida" is the problem. Couldn't it just say "Prior Pregnancies"?

Hello Idagrn,
It's wonderful to hear from you, I hope you had a great 3 day weekend! Thank you for taking the time to share your thoughts with us. When you are gathering her Health History, at the beginning of the appointment, and her Gravida up unto that point-- you do not yet know (for a pregnancy test) what the result is this would be PRIOR pregnancies. If she were to return for a sonogram the following week you absolutely could increment that by 1 to include the positive test as you now know she is pregnant. Does that make sense?

Blessings,
Ashleigh
